QMAP - Part Time- Assisted Living

We are seeking a dedicated and compassionate individual to join our team as a Qualified Medication Administration Personnel (QMAP). The QMAP is responsible for ensuring the safe and accurate administration of medications to residents under the direction of a licensed nurse. The ideal candidate will have a strong attention to detail, excellent communication skills, and a passion for providing high-quality care to our residents.

The available shift is:

  1. Part Time/PRN - Monday 7:00am - 11:00am

Job Summary

Assist in maintaining a physical, social and psychological environment that is conducive to the best interest and welfare of residents.

Administer or assist with self-administration of medication and treatments as prescribed by the health care provider and observe/report responses to existing care plans based on state regulations and completion of required training/certification.

Maintain cleanliness of medication supply room, audit medication carts and provide accurate accounting of all medications.

 Accountabilities

  • General
    • Model the Culture Change Principles of the Eden Alternative.
    • Understand and adhere to Resident Confidentiality and HIPPA regulations.
    • Attend in-service training, educational classes, and meetings as required.
    • Read and comprehend instructions, correspondence and memos.
    • Communicate and function in an interdisciplinary team.
    • Maintain a thorough working knowledge of Community Electronic Health Record software.
    • Maintain a thorough working knowledge of Community policies and procedures.
    • Perform basic mathematic procedures.
    • Make independent decisions when circumstances warrant such action.
    • Maintain the highest degree of personal and professional integrity.
    • Know and honor Resident Rights.
    • Communicate with residents in a respectful manner.
    • Respond to resident family/visitor requests in a timely and appropriate manner.
    • Provide effective and courteous service to all residents, guests and co-workers.
  • Medication Administration
    • Ensure medication room door and medication cart is locked at all times.
    • Maintain neat, clean and stocked medication cart.
    • Order, store and administer medications according to Community policies and procedures and current physician orders.
    • Complete narcotic count on each shift, immediately reporting any concerns to the Health Wellness Director.
    • Read the daily notes for information and changes at the beginning of each shift.
    • Disposal of expired or discontinued medications in a timely manner according to Community policies and procedures.
    • Obtain & record vital signs as directed on the Medication Administration Record [MAR] or vital sign log in resident’s electronic health record. 
    • Adhere to Universal Precautions and Infection Control procedures.
    • Maintain medication and specimen refrigerator temperature logs.
  • Observation, Documentation and Reporting
    • Regularly review resident care plans and medication orders to be aware of additions or changes to required support and assistance.
    • Complete Incident Reports per Community Policy and Procedure. 
    • Complete Short Term Observation charting in resident’s Electronic Health Record [EHR] when/if applicable.
    • Document support and assistance provided in the resident’s Electronic Health Record [EHR] when/if applicable.
    • Document and report resident and/or family requests for additional or changes in support/assistance in the resident’s Electronic Health Record [EHR] when/if applicable.
    • Document and report observations including unusual or significant changes in resident physical, mental or behavioral conditions in the resident’s Electronic Health Record [EHR] when/if applicable.
  • Resident Care
    • May provide hands on resident care to residents.
    • When performing resident care:
      • Accomplish with an awareness of dignity, personal preferences and choice.
      • Promote resident independence in accordance with care plan.
      • Comply with Community infection control guidelines.
  • Physical Demands/Working Conditions
    • Able to sit, stand, bend, lift, push, pull, stoop, walk, and move frequently during working hours.
    • Able to lift 40 lbs. floor to waist, lift 10 lbs. waist to shoulder, lift and carry 40 lbs., and push/pull 40 lbs.
    • Able to assist in the evacuation of residents.
  • Qualifications and Educational Requirements
    • Must be able to effectively communicate in the English language and follow written and verbal instructions.
    • High School Diploma or equivalent.
    • Qualified Medication Administration Person [QMAP] as required by Colorado regulations.
    • CPR certification which must remain current during employment.
    • First aid certification from a nationally recognized organization preferred.
    • Minimum of one year experience in a senior care or healthcare setting preferred.
